OVERVIEW

Hale Makua Kahului is a 254 bed skilled nursing facility offering an array of short-term and long-term services and Adult Day Health Care (temporarily closed due to COVID-19 restrictions).

Hale Makua Wailuku is a 124 bed skilled nursing facility also offering an array of services including long-term and short-term rehab services.

REHABILITATION

Hale Makua's Rehabilitation Team includes a team of experts to provide evaluation and treatment of both neurological and orthopedic disorders with a comprehensive multi-disciplinary approach. We offer physical, occupational, and speech therapy. We help patients recover from joint replacement procedures, fractures, strokes, pressure ulcers and acute illnesses. Services include discharge planning, family/caregiver training and assessments to assure a safe transition to home. In addition, our Restorative Nursing Program helps to maintain residents at their highest functioning level.

SKILLED & INTERMEDIATE (LONG-TERM) NURSING CARE

Registered and licensed nurses and certified nursing assistants provide 24 hour care. Services include medication management, food and nutritional evaluation, and specialized dietary support. Each resident is assessed and a plan of care is designed by an interdisciplinary team. This team includes the medical director, nurses, social worker(s), rehabilitation therapist(s), activities coordinator and dietitian. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments. Stimulating and meaningful social and recreational activities are offered daily to residents and adult day health participants alike.
Other specialized services include advanced wound care, diabetes support, IV therapy, enteral nutrition, and pain management.
