REHABILITATION
Ann Pearl's Rehabilitation Team provides evaluation and treatment of both neurological and orthopedic disorders with a comprehensive, multi-disciplinary approach. We offer inpatient physical, occupational and speech therapy. Services include discharge planning, family/caregiver training and assessments to assure a safe transition to home. Our Restorative Nursing Program helps to maintain residents at their highest functioning level.
SKILLED AND INTERMEDIATE (LONG-TERM) NURSING CARE
Registered and licensed nurses and certified nursing assistants provide 24 hour care. Services include medication management, food and nutritional evaluation and specialized dietary support. Each resident is assessed and a plan of care is designed by an interdisciplinary team. This team includes the medical director, nurses, social workers, rehab therapists, activities coordinator and dietician. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments. Stimulating and meaningful social and recreational activities are offered daily to residents and adult day health participants alike. Other specialized services include advanced would care, diabetes support, IV therapy, enteral nutrition and pain management.
HALE HO'OLU MEMORY CARE
Hale Ho‘olu is a specialized wing in Ann Pearl designed for the benefit of clients with Alzheimer’s disease and other types of dementia. This area provides a supportive, comfortable, safe and nonthreatening environment for long term residents as well as adult day stay participants.
Physical amenities include:
Multipurpose room and covered lanai
Secured courtyard with guided walkways and benches
Kitchenette and community dining area
We also offer a warm, caring and family-oriented atmosphere. Purposeful and meaningful activities are provided according to individual needs and interests. We encourage family participation as well.
ADULT DAY HEALTH
This program allows kupuna to live at home or in the greater community yet receive assistance with personal needs such as toileting, mobility, and medication management. Elders in this program can participate in a variety of social and therapeutic activities. This program also gives families a break from caregiving responsibilities. Lunch and snacks are included in the daily rate. Additional services and meals are available for an additional charge.
Adult Day Health hours are 7:00 AM - 5:30 PM Monday through Friday.
