Care Management

Caring for kūpuna (elderly) is one of society's most essential tasks, yet a fragmented health care system misses opportunities to ensure that our aging population is supported across the continuum of care. At Ohana Pacific Health, our aim is to provide kūpuna with care, support, and respect throughout their care journey.  

Needs are supported through transition of care support, health education, medical and medication management, and social services assistance using a culturally relevant approach.
 

Who is Kālele?

Kālele is here to help kūpuna (elders), Hawaii families, and their caregivers. Kālele also serves as an extension of other services, like rehabilitation, you may receive from Ohana Pacific Health or Hale Makua and provides guidance and support while speaking up for your needs.

Healthcare can be hard to understand. Kālele can help you:

  • Get medical and medication help at the right time and in the right place

  • Find helpful community resources

  • Stay healthy or get better after leaving a hospital or nursing home

 

Our team consists of:

  • Community Health Workers (CHW) - friendly guides or patient advocates

  • Medical providers

  • Other support staff
