Require intermittent skilled care such as Nursing, Physical, Speech or Occupational Therapy.

Have a referral or prescription for home health from your health care provider.

Our team of nurses and therapists will work closely with your health care provider to create a plan of care to help you make the best recovery possible. From helping you understand your medications and discharge instructions, to coordinating follow-up care and services, we can help keep you at home and out of the hospital.​

Are you having trouble getting around or are you afraid of falling?​

Our therapists can help you understand how to reduce the risk of falls and injuries, and coach you to be safe at home. We take an individualized approach to providing strategies designed to help improve your balance and mobility.​

​Are you living with a chronic condition such as COPD, diabetes, heart failure, kidney disease, or high blood pressure?

​Our clinicians will give you the information you need to take charge of your health. We give you the tools you need to spot any potential issues early and take action to avoid problems, giving you greater independence.

Do you need help understanding and managing your medications?

We teach you how to safely take the right dose at the right time and frequency, including any IV medications your health care provider may prescribe. Our nurses help you manage your medication schedule, and let your health care provider know how you are doing.

Do you need help with caring for a wound?

Our nurses can help you manage all your wound care needs including complex wound care, wound assessment, and teaching. We can also provide therapy to help you recover from your injury.​

HOW MUCH DOES HOME HEALTH COST?

Home health can often cost less than traditional hospital or nursing care. Most Medicare and Medicaid plans cover home health services prescribed with no co-payment.
