OVERVIEW

Hale Kupuna Heritage Home is an 84 bed skilled nursing and intermediate long term care center in Omao, on the island of Kauaʻi. Nestled in a residential neighborhood away from the hustle and bustle of the city, residents enjoy the natural, lush, green surroundings, while benefitting from the services we offer.

The secure and comfortable center provides a complete range of services for patients that are unable to return home after a hospitalization. Our Clinical Programs provide a safe transition from hospital to home with a depth of services. The objective of our transitional care program is to help our guests achieve their goal of reaching their full potential and getting home as soon as possible. Your care is managed through our Interdisciplinary Care Team of attending Physicians, Nurse Practitioners, nursing team, therapy teams, and social services.

Typical therapy treatments are from Monday to Friday. However, unlike many other nursing facilities, our therapy team is available seven days per week to maximize each guests recovery. The therapist will also identify any barriers or safety risks in the home setting. Family and guest education is geared to the specific diagnosis and our Social Worker will coordinate Outpatient Therapy or Home Health Services.

REHABILITATION

Hale Kupuna's Rehabilitation Team includes a team of experts to provide evaluation and treatment of both neurological and orthopedic disorders with a comprehensive multi-disciplinary approach. We offer physical, occupational, and speech therapy. We help patients recover from joint replacement procedures, fractures, strokes, pressure ulcers and acute illnesses. Family training and patient education is ongoing throughout our therapy sessions. Our therapists will help to identify any barriers or safety concerns within the home when preparing for discharge. Social Services collaborates with the therapy team to ensure our guests are prepared to return to the community safely. At Hale Kupuna, our recommendations for equipment, home health services and out patient therapies provides our guests with a secure plan for community reintegration. For our long term care residents, we closely monitor changes in strength and mobility, providing services as needed. Our Restorative Nursing Program keeps our residents functioning at their highest potential, giving them the best possible quality of life.

SKILLED & INTERMEDIATE (LONG-TERM) NURSING CARE

Registered and licensed nurses and certified nursing assistants provide 24 hour care. Services include medication management, food and nutritional evaluation, and specialized dietary support. Each resident is assessed and a plan of care is designed by an interdisciplinary team. This team includes the medical director, nurses, social worker(s), rehabilitation therapist(s), activities coordinator and dietitian. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments. Stimulating and meaningful social and recreational activities are offered daily to residents and adult day health participants alike.
Other specialized services include advanced wound care, diabetes support, IV therapy, enteral nutrition, and pain management.
