CARE MANAGEMENT

Kūpuna and caregiver support throughout the care journey

Our grassroots, community-led model leverages Community Health Navigators (CHNs) or non-licensed, non-clinical wellness advocates along with licensed clinical staff to provide a trusted resource in the community. Needs are supported through transition of care support, health education, chronic condition management, and social services assistance through a culturally relevant approach.

Caring for kūpuna is one of society's most essential tasks.

A fragmented health care system misses opportunities to ensure that our aging population is supported across the continuum of care. Kūpuna face daily challenges including chronic health issues, poverty, and access to food and social services.

Kālele Care Services is an advocate for kūpuna and a support system for caregivers. Navigating the health care system is a challenge for everyone. Our team provides comprehensive assistance in your care journey starting at bedside in the hospital until you are safely and happily back home.
