OVERVIEW

PuÊ»uwai Ê»O Makaha is a 93 bed skilled nursing facility offering an array of short-term and long-term services since 1979. Respite services are provided through  as well so families can take a short break from their caregiver duties. Respite Care includes help with medications, assistance with Activities of Daily Living and personalized care planning. Additionally, PuÊ»uwai Ê»O Makaha works collaboratively with hospice providers to provide nurturing and supportive care so residents may spend their final days in comfort.

REHABILITATION

Our Rehabilitation Team provides evaluation and treatment of both neurological and orthopedic disorders with a comprehensive multi-disciplinary approach. We offer inpatient physical, occupational, and speech therapy. Services include discharge planning, family/caregiver training and assessments to assure a safe transition to home. In addition, our Restorative Program helps to maintain residents at their highest functioning level.

SKILLED & INTERMEDIATE (LONG-TERM) NURSING CARE

Registered and licensed nurses and certified nursing assistants provide 24 hour care. Services include medication management, food and nutritional evaluation, and specialized dietary support. Each resident is assessed and a plan of care is designed by an interdisciplinary team. This team includes the medical director, nurses, social worker(s), rehabilitation therapist(s), activities coordinator and dietitian. Residents are monitored for changes in condition and plans of care are altered based on regular ongoing assessments. Stimulating and meaningful social and recreational activities are offered daily to the residents.
Other specialized services include advanced wound care, diabetes support, IV therapy, enteral nutrition, and pain management.
