HOW TO QUALIFY FOR SERVICES
Under the Care of a Licensed Physician or Provider who will assist with the development of a plan of care.
You must require either Skilled Nursing or Physical Therapy care.
Have a difficulty leaving your home, and thus require the care to be provided in your home.
The care required must be intermittent, and you are safe in your home when your home health care provider leaves your home.

Care is frequently provided to those:
Recently discharged from the Hospital
Elderly or Disabled Individuals that need help transition to their home after a prolonged illness
Wound or Ostomy Care, Care related to a feeding tube or other drainage tub
Chronic Illnesses that have gotten worse, and requires monitoring to avoid hospitalization
Intravenous Treatments and Therapy
Complex Conditions that require rehabilitation: strokes, heart attack, cancer care, etc
