OVERVIEW
Legacy Hilo Rehabilitation & Nursing Center is a newly built 100-bed, skilled rehab and nursing facility located in the heart of Hilo on the Big Island. We serve those who require clinical and rehabilitative care until they can safely go home or transition into a long-term care community. We strive to provide clinical excellence and genuine care through our multidisciplinary team approach.
REHABILITATION
Legacy Hilo’s Rehabilitation Team provides evaluation and treatment of both neurological and orthopedic disorders with a comprehensive multi-disciplinary approach. We offer inpatient physical, occupational, and speech therapy. Services include discharge planning, family/caregiver training, and assessments to assure a safe transition to home. In addition, our Restorative Nursing Program helps to maintain residents at their highest functioning level.
SKILLED & INTERMEDIATE (LONG-TERM) NURSING CARE
Legacy Hilo provides assistance with the activities of daily living, including personal hygiene, dressing, eating, walking, toileting, & transferring from one position to another. Planned events provide residents with an opportunity to enjoy the companionship of others in a warm & caring family-oriented setting. Registered and licensed nurses and certified nursing assistants provide 24-hour care. Services include medication management, food and nutritional evaluation, and specialized dietary support. Each resident is assessed and an individualized plan of care is designed by an interdisciplinary team. This team includes the medical director, in-house physician, licensed nurses, social worker(s), rehabilitation therapist(s), activities coordinator and dietitian. Clients are monitored for changes in condition and plans of care are altered based on regular ongoing assessments. Other specialized services include advanced wound care, diabetes support, IV therapy, enteral nutrition, and pain management. Whatever your situation requires, we coordinate the many components of your care.
